Form ST-21 Sales and Use Tax Refund Application

 

INSTRUCTIONS: KANSAS SALES AND USE TAX REFUND APPLICATION (Form ST-21)

 

 

When sales or use tax has been paid in error on an exempt transaction, you should apply for a refund using a form ST-21. This document is found on the website of the Kansas Department of Revenue. Note that all sections marked with an asterisk must be completed.

 

Kansas Sales And Use Tax Refund Application ST-21 Step 1: In Part A, give the name, address, city, state, zip code, employer identification number and state tax account number of the vendor. Also provide the name of a contact person and their telephone number. You may choose to provide their email address and fax number as well.

 

Kansas Sales And Use Tax Refund Application ST-21 Step 2: Indicate with a check mark whether a retailer is filing for tax they paid or for tax paid by their customer, or if the form is filed by a consumer filing for a refund of tax paid either to the vendor or directly to the state.

 

Kansas Sales And Use Tax Refund Application ST-21 Step 3: In Part B, the name, address, and employer identification number or Social Security number of the customer should be provided. Provide the name and daytime telephone number of a contact person for them as well.

 

Kansas Sales And Use Tax Refund Application ST-21 Step 4: In Part C, enter the size of the refund you are seeking. 

 

Kansas Sales And Use Tax Refund Application ST-21 Step 5: Check the box next to the type of refund you are seeking.

 

Kansas Sales And Use Tax Refund Application ST-21 Step 6: Part D consists of a table on the second page. For every purchase you are seeking a refund for, enter an invoice date and number, the name of the vendor on the invoice, a description of the item or items in question, the retail price, the tax paid, the account code, and a written explanation for the exemption. Total the refund requested at the bottom of the chart.

 

Kansas Sales And Use Tax Refund Application ST-21 Step 7: Part E is to be completed by the retailer. It authorizes the customer to file their request directly with the Kansas Department of Revenue. If the retailer cannot be located, is out of business or the retailer does not respond to the customer's request, they must complete part F.
