INSTRUCTIONS: OHIO CERTIFICATE OF TERMINATION
To terminate administration of an Ohio estate, file the certificate discussed in this article. This document can be obtained from the website maintained by the Supreme Court of Ohio.
Ohio Certificate Of Termination Step 1: Enter the name of the county in which the probate court overseeing the case is located on the first blank line.
Ohio Certificate Of Termination Step 2: Enter the name of the estate on the second blank line.
Ohio Certificate Of Termination Step 3: Enter the assigned case number on the third blank line.
Ohio Certificate Of Termination Step 4: Line 1 states that all debts and claims presented to the estate have been paid in full or settled finally.
Ohio Certificate Of Termination Step 5: Line 2 states that an estate tax return, if required under Chapter 5731 of the Revised Code, has been filed, and that any estate tax due under that chapter has been paid.
Ohio Certificate Of Termination Step 6: On line 3, check the first box if attorney fees have been waived by counsel of record.
Ohio Certificate Of Termination Step 7: Check the second box on line 3 if attorney fees have been paid to counsel of record and enter the amount paid.
Ohio Certificate Of Termination Step 8: On line 4, check the first box if fiduciary fees have been waived.
Ohio Certificate Of Termination Step 9: On line 4, check the second box if the fiduciary has been paid. If so, enter the amount paid.
Ohio Certificate Of Termination Step 10: Line 5 states that all assets remaining after completion of the activities described above have been distributed to the fiduciary as sole legatee, devisee or heir.
Ohio Certificate Of Termination Step 11: Enter the name of the attorney for the fiduciary on the next blank line.
Ohio Certificate Of Termination Step 12: Enter the name of the fiduciary on the next blank line.
Ohio Certificate Of Termination Step 13: Enter the registration number of the attorney on the next blank line.
Ohio Certificate Of Termination Step 14: Submit the form to the probate court which is supervising the case.
Ohio Certificate Of Termination Step 15: After reviewing the certificate of termination, the probate judge overseeing the case will enter their signature on the last blank line.
