INSTRUCTIONS: NEVADA APPLICATION FOR SALES/USE TAX EXEMPTION FOR RELIGIOUS/CHARITABLE/EDUCATIONAL ORGANIZATIONS (Form APP-02.01)

 

 

Nevada organizations which are religious, charitable or educational may apply for an exemption on payments of sales or use tax for purchases of tangible personal property or services for exempt purposes. This is done by filing a form APP-02.01, which is found on the website of the Nevada Department of Taxation which processes this form. Note that the requirements for exemption from state sales and use taxes are not automatically the same as those qualifying an organization for a federal tax exemption. Be sure to read the laws concerning eligibility included in the instructions.

 

Nevada Application For Sales/Use Tax Exemption For Religious/Charitable/Educational Organizations APP-02.01 Step 1: At the top of the form, indicate with a check mark whether you are filing a new application or applying for a renewal of a previously granted exemption.

 

Nevada Application For Sales/Use Tax Exemption For Religious/Charitable/Educational Organizations APP-02.01 Step 2: In the space provided, give the name of your organization, its telephone number and physical address. Enter the mailing address if different. Give a daytime phone number and fax number if applicable.

Nevada Application For Sales/Use Tax Exemption For Religious/Charitable/Educational Organizations APP-02.01 Step 4: Write a brief description of the primary function of your organization. 

 

Nevada Application For Sales/Use Tax Exemption For Religious/Charitable/Educational Organizations APP-02.01 Step 5: Below this, indicate with a check mark all types of substantiating documentation you are attaching. Those listed included copies of your organization's by laws or articles of incorporation, financial statements from the previous 12 months documenting revenue sources and expenditures, exemption letters from the IRS, or any other relevant information. 

 

Nevada Application For Sales/Use Tax Exemption For Religious/Charitable/Educational Organizations APP-02.01 Step 6: If filing for a charitable organization, an outline of your organization's charitable activities, fund raisers goals and a business plan must be attached. If filing for a voluntary organization, give a list of all such activities from the past 12 months and the number of hours devoted to them. Sign and date the form.
