INSTRUCTIONS: MONTANA POWER OF ATTORNEY PACKET

This article discusses a packet made available by the Montana Department of Public Health and Human Services for those who wish to confer an agent with power of attorney. A sample power of attorney form which can be used begins on the seventeenth page.

Montana Power Of Attorney Packet Step 1: Enter your name on the first blank line.

Montana Power Of Attorney Packet Step 2: Enter your mailing street address on the second blank line.

Montana Power Of Attorney Packet Step 3: Enter your city, state and zip code on the third blank line.

Montana Power Of Attorney Packet Step 4: Enter your name and address on the fourth blank line.

Montana Power Of Attorney Packet Step 5: Enter the name and address of the person being appointed on the fifth blank line.

Montana Power Of Attorney Packet Step 6: On lines A through N, enter your initial next to all powers you wish to grant to the agent being appointed. You may choose to strike through any powers which you do not wish to grant, but this is not required.

Montana Power Of Attorney Packet Step 7: In the blank box, where indicated, enter any special instructions concerning expansions or limitations of power for the agent being appointed.

Montana Power Of Attorney Packet Step 8: Below the blank box, strike through the appropriate statement if you do not wish for this form to revoke all previous powers of attorney.

Montana Power Of Attorney Packet Step 9: On the next page, strike through the appropriate statement if you do not wish for this power of attorney to remain in effect of your becoming disabled, incapacitated or incompetent.

Montana Power Of Attorney Packet Step 10: If you do not wish for the agent to become your conservator or guardian, strike through the applicable statement.

Montana Power Of Attorney Packet Step 11: Where indicated, you may enter up to three successors to the agent in the event they become incompetent, resign or refuse to accept the office of agent.

Montana Power Of Attorney Packet Step 12: On the last page, enter the date on the first three blank lines.

Montana Power Of Attorney Packet Step 13: Enter your signature, the county and the date in the presence of a notary public, who will affix their seal.
