Special Affidavit of Qualification (Surety Bond-General Affidavit Filed and Current)
This form is used when a General Affidavit of Qualifications (Attorney-In-Fact) form and General Affidavit-Questionnaire (Bondsman) form is filed with the court.
This form must be filed with every bond written during the month covered by the general affidavit.
