Form 128 Application for Abatement of Real or Personal Property Tax

 

INSTRUCTIONS: MASSACHUSETTS APPLICATION FOR ABATEMENT OF REAL OR PERSONAL PROPERTY TAX (Form 128)

 

 

Massachusetts residents who feel that the property tax payment that has been assessed by the state is excessive and unfair may file a form 128 to request an abatement. You may be required to pay part or all of your assessed tax before you will be able to pay this form. This document is found on the website of the Massachusetts Department of Revenue. 

 

Massachusetts Application For Abatement Of Real Or Personal Property Tax 128 Step 1: At the top of the first page, give the name of the city or town in which you are filing and the fiscal year involved. 

 

Massachusetts Application For Abatement Of Real Or Personal Property Tax 128 Step 2: Indicate with a check mark whether you are seeking an abatement of your real or personal property tax.

 

Massachusetts Application For Abatement Of Real Or Personal Property Tax 128 Step 3: Give your name and address in the blank above "instructions."

 

Massachusetts Application For Abatement Of Real Or Personal Property Tax 128 Step 4: In section a, give the name of the owner. Give the name of the applicant if the person completing the form is not the property owner. Indicate their status with a check mark and give their mailing address and telephone number. Give the amounts and dates of tax payments in question.

 

Massachusetts Application For Abatement Of Real Or Personal Property Tax 128 Step 5: In section B, use your tax bill to provide the tax bill number, the assessed valuation, the location of the property and a description of the property.

 

Massachusetts Application For Abatement Of Real Or Personal Property Tax 128 Step 6: If real property is concerned, give the parcel ID, land area size and class.

 

Massachusetts Application For Abatement Of Real Or Personal Property Tax 128 Step 7: If personal property is concerned, list the property type or types.

 

Massachusetts Application For Abatement Of Real Or Personal Property Tax 128 Step 8: In section C, indicate whether you are seeking an abatement due to an overvaluation, incorrect usage classification, disproportionate assessment or another reason. If the later, specify. Give your own assessment of the value, the class of the property and provide a written explanation for your disputation. Sign and date the second page.
