INSTRUCTIONS: NEW YORK SUPPORT (Form 4-3a)
In cases where a New York parent ordered to pay child support has failed to maintain their financial obligations, you may file a complaint in court. The Form 4-3a discussed in this article is filed to request that an order be issued concerning payments. This document is available on the website of the New York Courts system.
New York Support 4-3A Step 1: At the top of the page, enter the county in which you are filing.
New York Support 4-3A Step 2: Enter the case name, docket number, and the names of all parties involved where indicated.
New York Support 4-3A Step 3: In section one, enter the name of the county in which you are filing this petition.
New York Support 4-3A Step 4: In section three, give the date and location of your wedding.
New York Support 4-3A Step 5: In section four, give the names and dates of birth of all spouses and children on whose behalf support is being sought.
New York Support 4-3A Step 6: In section five, give the date on which the respondent stopped making support payments.
New York Support 4-3A Step 7: In section six, indicate with a check mark or written explanation why the respondent is unlikely to make payments.
New York Support 4-3A Step 8: In section seven, indicate with a check mark or written explanation what the respondent's source of income is.
New York Support 4-3A Step 9: Print and sign your name on the fourth page. If you have an attorney, they should also sign and print their name. Additionally, they should include their address and telephone number.
