INSTRUCTIONS: MONTANA CHILD SUPPORT GUIDELINES FINANCIAL AFFIDAVIT
During family court cases in Montana which concern minor children, you may be required to submit an affidavit documenting your fiscal circumstances. This document will help the court rule on issues concerning child support payments. The form can be found on Montana's official state website, in the section concerning the state's Department of Health and Human Services.
Montana Child Support Guidelines Financial Affidavit Step 1: In section A, give all personal information requested, including your name, address, contact information, education history and tax filing status.
Montana Child Support Guidelines Financial Affidavit Step 2: Section B concerns your children. Give all information requested about minor children, including their names and ages. Note any support you are already paying. Give all information requested about any health insurance you have through your job.
Montana Child Support Guidelines Financial Affidavit Step 3: Section C concerns your job. Give all information requested about your current employers, the nature of your work and any unemployment benefits you are currently recieving.
Montana Child Support Guidelines Financial Affidavit Step 4: In section D, detail all your sources of income. You should include copies of your last three pay stubs, as well as all tax forms filed for the last three years.
Montana Child Support Guidelines Financial Affidavit Step 5: Section E concerns deductions withheld from your wages, as well as other expenses.
Montana Child Support Guidelines Financial Affidavit Step 6: In section F, list any changes you expect in your life or that of your children in the next 18 months that would affect your ability to pay child support.
Montana Child Support Guidelines Financial Affidavit Step 7: Include any additional comments in section F as necessary.
Montana Child Support Guidelines Financial Affidavit Step 8: The form must be signed and dated in the presence of a notary public. Include the state and county you are filing in.
