California Petition For Appointment Of Probate Conservator GC-310 Step 1: In the first box at the top of the page, enter the name of your attorney, their State Bar number and address and the name of the person being represented, as well as their telephone number. If you are not represented by an attorney, provide your name, address and telephone number. In either instance, providing a fax number or email address is optional.
