List the name of the limited partnership on the first line of this form. Make sure the name contains one of the following words or abbreviations: limited partnership, limited, or ltd.

Provide the address for the limited partnership’s principal place of business in the next section. Make sure you include the city, state and zip code.

If you want to delay the effective date after the Secretary of State approves the limited partnership, provide an effective date on this form no later than 90 days after filing.

Provide the name and business or residential address for each general partner at the bottom of the first page. If additional space is needed, attach an additional sheet.
