INSTRUCTIONS: ARKANSAS IFTA APPLICATION (Form AMFT-71)
To register for the International Fuel Trade Agreement in Arkansas, a form AMFT-71 should be filed. This document can be obtained from the website maintained by the Arkansas Department of Finance and Administration.
Arkansas IFTA Application AMFT-71 Step 1: At the top right-hand corner of the page, enter your registration year.
Arkansas IFTA Application AMFT-71 Step 2: In box 1, enter your federal employer identification number or Social Security number.
Arkansas IFTA Application AMFT-71 Step 3: In box 2, enter your Arkansas IRP account number, as well as its expiration date.
Arkansas IFTA Application AMFT-71 Step 4: In box 3, enter your United States Department of Transportation number.
Arkansas IFTA Application AMFT-71 Step 5: In box 4, enter your legal name.
Arkansas IFTA Application AMFT-71 Step 6: In box 5, indicate with your check mark whether this is an original, renewal or supplemental application.
Arkansas IFTA Application AMFT-71 Step 7: In box 6, enter your trade or "doing business as" name (if different from your legal name).
Arkansas IFTA Application AMFT-71 Step 8: In box 7, enter your Arkansas phone number, including the area code.

Arkansas IFTA Application AMFT-71 Step 12: In box 11, enter the telephone number of a contact person, including the area code.
Arkansas IFTA Application AMFT-71 Step 13: In box 12, indicate with a check mark whether the business is managed by a sole proprietor, by a partnership, or by a corporation.
Arkansas IFTA Application AMFT-71 Step 14: In box 13, give the name, title and physical residence address of two partners or corporate officers.
Arkansas IFTA Application AMFT-71 Step 15: In box 14, list jurisdictions in which you have bulk storage.
Arkansas IFTA Application AMFT-71 Step 16: In box 15, give the number of vehicles which will require IFTA decals.
Arkansas IFTA Application AMFT-71 Step 17: On the first blank line at the bottom left corner of the page, enter your signature.
Arkansas IFTA Application AMFT-71 Step 18: On the second blank line, enter your title and the date.
