INSTRUCTIONS: ARKANSAS COMBINED REGISTRATION CHANGE FORM (Form AR-40)
To change Arkansas tax account information, file a form AR-40. This document is meant to be completed on the website of the Arkansas Department of Finance and Administration.
Arkansas Combined Registration Change Form AR-40 Step 1: In the first box, enter your federal employer identification number or Social Security number.
Arkansas Combined Registration Change Form AR-40 Step 2: In the second box, enter the legal name of your business.
Arkansas Combined Registration Change Form AR-40 Step 3: In the third box, enter the current legal name of your business.
Arkansas Combined Registration Change Form AR-40 Step 4: In the fourth box, enter the new legal name of your business.

Arkansas Combined Registration Change Form AR-40 Step 9: The next six boxes allow you to document changes to your account. In the first column, select the type of tax account you are changing by choosing the appropriate option from the pull-down column.
Arkansas Combined Registration Change Form AR-40 Step 10: In the second column, indicate with a check mark whether you are changing the associated address, doing business as name or location.
Arkansas Combined Registration Change Form AR-40 Step 11: In the third column, give the new account address or doing business as name.
Arkansas Combined Registration Change Form AR-40 Step 12: The next section concerns contact information. You may document up to two contact people. On the first line of each section, indicate whether you are adding, changing or deleting the person whose information you are entering.
