INSTRUCTIONS: ARIZONA TRANSACTION PRIVILEGE TAX EXEMPTION CERTIFICATE (Form 5000)

 

 

Qualified Arizona purchasers must provide a vendor with a completed form 5000 in order not to pay sales and use tax. This document can be found on the website of the Arizona Department of Revenue.

 

Arizona Transaction Privilege Tax Exemption Certificate 5000 Step 1: In the section where indicated, give the name and address of the purchaser. Give their vendor name.
