Arizona Tax Clearance Application 25-002 Step 9: In the next section, indicate with a check mark if you are applying for a letter of good standing concerning gaming, a healthy forest certification, motion picture production incentive, personal letter, renewable energy tax initiative, residency, sale of business or other. If the latter, explain.
Arizona Tax Clearance Application 25-002 Step 10: Section 3 concerns the type of application being filed. Check the applicable box next to the first six statements if filing as a corporation, S corporation, partnership, tax exempt organization, limited liability company or limited liability partnership. Provide your federal employer identification number or taxpayer identification number, Arizona transaction privilege license number and Arizona withholding tax license number.
Arizona Tax Clearance Application 25-002 Step 11: Check the applicable statement if filing as an estate, trust or individual. Enter your federal employer identification number or Social Security number, Arizona transaction privilege license number and Arizona withholding tax license number.
Arizona Tax Clearance Application 25-002 Step 12: In section 4, print your name on the first blank line, print your specific title on the second blank line, enter your signature on the third blank line and enter the date on the fourth blank line.
