Form PT-021 Annual Statement for Continued Property Tax Exemption

INSTRUCTIONS: UTAH ANNUAL STATEMENT FOR CONTINUED PROPERTY TAX EXEMPTION (Form PT-21)

 

Utah non-profit organizations which file a form PT-20 seeking property tax exemptions for real property used for religious, charitable or educational purposes can file a form PT-21 on an annual basis to extend this status. This form is available on the website of Utah's Property Tax division.

 

Utah Annual Statement For Continued Property Tax Exemption PT-21 Step 1: Under the section headed "general information," give the name of the organization. This name must be the recorded owner of the property in question.

 

Utah Annual Statement For Continued Property Tax Exemption PT-21 Step 2: Enter the full address of the organization.

 

Utah Annual Statement For Continued Property Tax Exemption PT-21 Step 3: Enter the name of a contact person or authorized representative and their telephone number. 

 

Utah Annual Statement For Continued Property Tax Exemption PT-21 Step 4: Give the location of the property in question, including the county, its parcel, account or serial number, the current tax year, and the year in which the exemption was originally issued.

 

Utah Annual Statement For Continued Property Tax Exemption PT-21 Step 5: Question one asks for a yes or no answer as to whether there have been any changes in use of the property since January 1 of the previous year. If yes, provide a written explanation.

 

Utah Annual Statement For Continued Property Tax Exemption PT-21 Step 6: Question two asks whether any individual or entity conducts for-profit businesses on this property. If yes, give their name and address as well as an explanation of their business.

 

Utah Annual Statement For Continued Property Tax Exemption PT-21 Step 7: Question three asks if any other organization uses this property and pays fees in excess of the costs of maintenance and utilities. If yes, give their name, address and the use they make of the facilities.

 

Utah Annual Statement For Continued Property Tax Exemption PT-21 Step 8: Question four asks whether any personal property on the real property is leased or rented. If so, give the name and address of the owner, as well as the type, make, model and serial number of the property.

 

Utah Annual Statement For Continued Property Tax Exemption PT-21 Step 9: The owner of record or their authorized representative should sign the form, enter the date and state their position.
