INSTRUCTIONS: TENNESSEE REVISED STATE AND LOCAL SALES AND USE TAX RETURN (Form SLS-450) Tennessee businesses must file a form SLS-450 on the 20th of every month following their reporting period to document all sales and use taxes owed to both the state Department of Revenue and in their local county or municipality. The form is found on the website of the Tennessee Department of Revenue. A separate instruction packet is available at the same location. Tennessee Revised State And Local Sales And Use Tax Return SLS-450 Step 1: At the top of the form, enter the filing period, the due date for this return, your business account number and the address of your business. If this is an amended return, check the box at the right. Tennessee Revised State And Local Sales And Use Tax Return SLS-450 Step 2: In line 1, provide your gross sales for the period. In line 2, provide the total cost of personal property bought on a resale certificate and used by you. In line 4, give the fair market value of all tangible personal property derived from the earth for use in the state, such as produce. Add lines 1 through 4 and enter the total in line 5. Transfer the value from line K to line 6 of the first page. Follow the instructions to complete lines 7 through 10.
