INSTRUCTIONS: NEW MEXICO APPLICATION FOR REFUND (Form RPD-41071)
New Mexico residents may use a form RPD-41071 to apply for a refund on overpaid taxes by filing a form RPD-41071. Note that you may also do so using a letter of your own as long as it contains all the same information on this form. The document can be found on the website of the New Mexico Taxation and Revenue Department.
New Mexico Application For Refund RPD-41071 Step 1: On the first line, enter your name and your taxpayer identification number.
New Mexico Application For Refund RPD-41071 Step 2: On the second line, enter your mailing street address.
New Mexico Application For Refund RPD-41071 Step 3: On the third line, enter your city, state and zip code.
New Mexico Application For Refund RPD-41071 Step 4: On the fourth line, give the name of a contact person and their phone number.
New Mexico Application For Refund RPD-41071 Step 5: In the first blank below this, write out the sum of the overpayment made.
New Mexico Application For Refund RPD-41071 Step 6: In the next blank, give the size of the overpayment in numerical form.
New Mexico Application For Refund RPD-41071 Step 7: In the next blank, name the type of tax that was overpaid.
New Mexico Application For Refund RPD-41071 Step 8: In the next two blanks, give the starting and ending dates of the payment period in question.
New Mexico Application For Refund RPD-41071 Step 9: In the section below, provide a written explanation of the basis for your refund application. Attach additional sheets as necessary.
New Mexico Application For Refund RPD-41071 Step 10: Sign and date the bottom of the form and give your title if signing as an authorized agent. Type or print your name and give your phone number.
New Mexico Application For Refund RPD-41071 Step 11: If you wish to receive your refund in the form of a direct deposit, give your routing and account number. Indicate with a check mark whether this is a checking or savings account.
