INSTRUCTIONS: NEW HAMPSHIRE INTEREST AND DIVIDENDS TAX RETURN (Form DP-10) Those who New Hampshire state tax on interest on dividends should document and file a state form DP-10. This form is found on the website of the New Hampshire Department of Revenue Administration. New Hampshire Interest And Dividends Tax Return DP-10 Step 1: At the top of the form, enter the beginning and ending dates of your fiscal year if not filing on a calendar year basis.
