INSTRUCTIONS: MISSOURI INDIVIDUAL INCOME TAX RETURN — LONG FORM (Form MO-1040) Individuals who owe income tax to the state of Missouri can file a long-form MO-1040 return. This document can be found on the website of the Missouri Department of Revenue. Missouri Individual Income Tax Return — Long Form MO-1040 Step 1: At the top of the first page, enter your Social Security number, as well as that of your spouse if filing jointly. Provide your full name, and that of your spouse if applicable. If the form is being filed on behalf of a taxpayer by an attorney, accountant or another representative, they should enter their name where indicated.
