The form discussed in this article is used to request that an Ohio estate be relieved from administration. This document can be obtained from the website maintained by the Ohio Supreme Court.
Ohio Application To Relieve Estate From Administration 5.0 Step 1: On the first blank line, enter the name of the county in which the probate court is located.
Ohio Application To Relieve Estate From Administration 5.0 Step 2: On the second blank line, enter the name of the decedent.
Ohio Application To Relieve Estate From Administration 5.0 Step 3: On the third blank line, enter the assigned case number.
Ohio Application To Relieve Estate From Administration 5.0 Step 4: On the fourth blank line, enter the date of death of the decedent.
Ohio Application To Relieve Estate From Administration 5.0 Step 5: On the fifth blank line, enter the street address of the domicile of the decedent.
Ohio Application To Relieve Estate From Administration 5.0 Step 6: On the sixth blank line, enter the city, village or township and county of the domicile of the decedent.
Ohio Application To Relieve Estate From Administration 5.0 Step 7: On the seventh blank line, enter the post office, state and zip code of the domicile of the decedent.
Ohio Application To Relieve Estate From Administration 5.0 Step 8: Check the first statement if the decedent's will has been administered to probate in this court.
Ohio Application To Relieve Estate From Administration 5.0 Step 9: Check the second statement if it is believed the decedent did not leave a will.
Ohio Application To Relieve Estate From Administration 5.0 Step 10: Check the box next to the applicable statement below regarding the value of the assets concerned and the date of the death of the decedent.
Ohio Application To Relieve Estate From Administration 5.0 Step 11: On the left hand side of the bottom of the page, enter the applicant's attorney's signature, typed or printed name, address, phone number and registration number.
Ohio Application To Relieve Estate From Administration 5.0 Step 12: On the right hand side, enter your signature, printed or typed name, address and phone number. Complete the second page as instructed.
