INSTRUCTIONS: OHIO APPLICATION FOR APPORTIONMENT OF FAMILY ALLOWANCE (Form 7.2)
The form discussed in this article is used by an Ohio fiduciary administering an estate to request an apportionment of family allowance from an estate in cases where there is a surviving spouse and children of the decedent who are not the children of the surviving spouse, or in cases where there is no surviving spouse but there is more than one minor child of the decedent. This document can be obtained from the website maintained by the Ohio Supreme Court.
Ohio Application For Apportionment Of Family Allowance 7.2 Step 1: Enter the name of the county in which the probate court is located on the first blank line.
Ohio Application For Apportionment Of Family Allowance 7.2 Step 2: Enter the name of the decedent whose estate is being administered on the second blank line.
Ohio Application For Apportionment Of Family Allowance 7.2 Step 3: Enter the case number assigned to administration of the estate on the third blank line.
Ohio Application For Apportionment Of Family Allowance 7.2 Step 4: On the fourth blank line, if applicable, the name and address of the surviving spouse should be entered.
Ohio Application For Apportionment Of Family Allowance 7.2 Step 5: The remainder of the first page is for documentation of minor children. Up to five minor children may be documented on this page. On the first line of the section for each child, enter their name, address and date of birth.
Ohio Application For Apportionment Of Family Allowance 7.2 Step 6: On the second line of the section for each child, enter the name of their parent or custodian, indicate which by circling the appropriate label, and provide their address.
