INSTRUCTIONS: MONTANA LETTERS
This article discusses the form filed to document the letters of appointment for a personal representative in a montana estate case. This document can be obtained from the website maintained by the Montana Courts system.
Montana Letters Step 1: At the top left-hand corner, enter your name where indicated.
Montana Letters Step 2: Enter your address where indicated.
Montana Letters Step 3: Enter your phone number, including the area code, where indicated.
Montana Letters Step 4: Enter the name of the personal representative where indicated.
Montana Letters Step 5: On the first blank line, enter the number of the district where the county is located.
Montana Letters Step 6: On the second blank line, enter the name of the county where the court is located.
Montana Letters Step 7: On the third blank line, enter the name of the deceased person.
Montana Letters Step 8: To the right of the third blank line, enter the probate number.
Montana Letters Step 9: On the fourth blank line, enter the name of the person being appointed as personal representative.
Montana Letters Step 10: On the fifth blank line, enter the month and date.
Montana Letters Step 11: On the sixth blank line, enter the last two digits of the year.
Montana Letters Step 12: On the seventh blank line, enter the date of the day of appointment.
Montana Letters Step 13: On the eighth blank line, enter month of appointment.
Montana Letters Step 14: On the ninth blank line, enter the last two digits of the year of appointment.
Montana Letters Step 15: On the tenth blank line, the clerk of the court will enter their signature.
Montana Letters Step 16: On the eleventh blank line, the deputy clerk will enter their signature.
Montana Letters Step 17: On the twelfth blank line, the personal representative will enter the county. To the right of this, their Social Security number should be entered.
Montana Letters Step 18: On the thirteenth blank line, print your name.
Montana Letters Step 19: On the fourteenth blank line, enter the name of the deceased.
Montana Letters Step 20: On the fifteenth blank line, enter your signature.
Montana Letters Step 21: On the remainder of the form, the date will be entered, as well as the signatures of both the clerk of court and the deputy clerk.
