Form MN NAM102 Application for Name Change

INSTRUCTIONS: APPLICATION FOR NAME CHANGE (MN NAM102)

 

The application and other required forms are filed with the District Court in the county the applicant currently resides.  The following forms are also required:

·  Criminal History Check Release NAM103

·  Proposed Order Granting Name Change NAM107

·  Notice to Persons Who Have Had Their Name Changed NAM106

If you’ve been convicted of a felony and you want to change your name, you need to file Felon Name Change Notice to Prosecutors NAM104 and notify the authority that prosecuted you.

In the heading, only list your County.  The Clerk will fill out the rest.  In the first set of lines, provide the current legal names of the adults requesting a name change.  In the second set of lines, provide the new name of the applicant(s).

In part 2, provide the current address of the applicant(s).  In part 3, provide the current legal name of the applicant(s) and their date of birth.

In part 4, provide the name of the applicant’s spouse if applicable.  Check the appropriate box.

In part 5, provide information for the applicant’s minor children if there are children.  Check the appropriate box.  If a parent is also requesting a name change for a minor, they also need to fill out an application for the minor.

If a parent is requesting a name change for a minor or there is a minor child in any case, provide the name of the non-applicant parent in part 6.  In part 7, check the appropriate boxes and provide the new name on the line provided.

In part 8, provide information about any criminal background.  If the applicant has a criminal background, provide the appropriate information.  List information about each party’s property in the state of Minnesota, and check any boxes that apply in the following sections.

Before filing your application and other forms, an adult needs to undergo a federal criminal background check.  Bring all completed forms to the Clerk’s Office in your County and pay the filing fees.  Ask the clerk for more information.
