INSTRUCTIONS: KANSAS APPLICATION AND AFFIDAVIT FOR MARRIAGE LICENSE

To marry in the state of Kansas, a marriage license must be obtained. A Kansas Application and Affidavit from Marriage License can be downloaded and filled out in advance from the official website of the Kansas court system or at your local District Court office. However, the form must be signed by both the bride and groom in person before a court official.

Kansas Application And Affidavit For Marriage License Step 1: The top of the form is for the completion of the court officer officiating. Do not fill in any part of the application above the first full paragraph.

Kansas Application And Affidavit For Marriage License Step 2: Enter the county where the marriage is to take place where indicated.

Kansas Application And Affidavit For Marriage License Step 3: Enter the full name of the groom, as well as the city and state where he resides.
