Form IL Change of Name Packet

INSTRUCTIONS: IL CHANGE OF NAME PACKET

 

The forms in this packet need filed in Circuit Court where the petitioner currently resides.  The circuits are listed on page 4 of the packet.  Depending on your circumstances, you may need to fill out the following forms:

·  Application to Sue as a Poor Person

·  Motion to Waive Publication Costs

·  Order Waiving Publication Costs

·  Notice of Filing of Petition for Change of Name

·  Petition for Change of Name

·  Order for Change of Name

·  Letter to Newspaper

“IL Change of Name Packet Step 1”

If you cannot pay filing fees or publication costs for the change of name, you have to complete the following forms:

·  Application to Sue as a Poor Person

·  Motion to Waive Publication Costs

·  Order Waiving Publication Costs

The instructions for these forms are found on page 5 of the packet. 

“IL Change of Name Packet Step 2”

In the Notice, put the date of your hearing in the first blank.  The date of the hearing must be at least six weeks after the date you sent notification to the newspaper. In the second blank, print your current name.  In the third blank, provide your new name.  Provide the city where the court is located in the fourth blank. 

“IL Change of Name Packet Step 3”

The Letter to Newspaper is fairly straightforward.  Make sure to place your case name and case number in the second and third blank.  Provide your signature and contact information, and make sure to provide a stamped, self-addressed envelope.

In the Order, print your current legal name in the first blank.  In the second blank, provide your new name.

In Petition, place your current name in the first blank and your age in the second blank.  Provide your current address along with city and state in the third blank.  In the fourth blank, provide the date you started living in Illinois.  In the fifth blank, make sure you check the box indicating you’ve never been convicted of a felony.  You cannot file for a name change if you were ever convicted of a felony.  The rest of the form is simple to complete.

Make sure you make copies of all the forms and bring originals and copies to the clerk.  Sign the forms in front of Notary Public.
