INSTRUCTIONS: CALIFORNIA INCOME AND EXPENSE DECLARATION (Form FL-150) Divorcing couples who are resolving alimony and lawyer's fees issues are required to individually complete a California Income and Expense Declaration. This Form FL-150 can be downloaded from the California Courts system. California Income And Expense Declaration FL-150 Step 1: At the top of the form, enter your name and address or that of your attorney, the county and location of the court where your case is being heard, and the name of your other spouse (the "respondent"). California Income And Expense Declaration FL-150 Step 2: In section 1, provide all information requested about your current place of employment. You should attach copies of all pay stubs from the last two months. People with more than one job should detail all sources of income on a separate blank piece of paper.
