To request driver education supplies from the Connecticut Department of Motor Vehicles for a commercial, secondary or vocational school, use a form R-319. This document can be obtained from the website of the government of Connecticut.

Connecticut Driver Education Supplies Request R-319 Step 3: Enter the date of your request in the third blank box.
Connecticut Driver Education Supplies Request R-319 Step 4: Enter the name of the school or AARP unit in the fourth blank box.
Connecticut Driver Education Supplies Request R-319 Step 5: Enter the school license number, if applicable, in the fifth blank box.
Connecticut Driver Education Supplies Request R-319 Step 6: Enter the school telephone number in the sixth blank box.
Connecticut Driver Education Supplies Request R-319 Step 7: Enter your street address, city or town, state and zip code in the seventh blank box.
Connecticut Driver Education Supplies Request R-319 Step 8: The first form listed, the Driver Education Certificate-Commercial/Secondary Driving School (form CS-1) can only be requested by driving schools. Enter the amount requested in the blank column.
