INSTRUCTIONS: 2011 COLORADO PROPERTY TAX/RENT/HEAT REBATE APPLICATION (Form DR 104 PTC) Full-time Colorado residents whose total income is less than $12,313 if single and $16.025 if married are eligible for a property tax, rent or heat rebate as long as they are also 65 or older, 58 or older and a surviving spouse, or a disabled person of any age. You must also have paid your taxes for rent, heat and property to be eligible and cannot be a dependant on someone else's form. This document can be found on the website of Colorado. 2011 Colorado Property Tax/Rent/Heat Rebate Application DR 104 PTC Step 1: At the top of the form enter your name, date of birth and Social Security number, as well as that of your spouse if filing jointly.
