INSTRUCTIONS: COLORADO ORDER APPOINTING COURT VISITOR (Form JDF 809)

 

 

The form discussed in this article is issued by a Colorado court to appoint a visitor to investigate allegations made in a petition for the appointment of a conservator or guardian. This document can be obtained from the website maintained by the Colorado Judicial Branch.

Colorado Order Appointing Court Visitor JDF 809 Step 10: Check the first box if the visitor is appointed to investigate allegations made in a petition for appointment of a guardian.

 

Colorado Order Appointing Court Visitor JDF 809 Step 11: Check the first box if the visitor is appointed to investigate allegations made in a petition for appointment of a conservator.

 

Colorado Order Appointing Court Visitor JDF 809 Step 12: If a hearing has been set, enter its date time, courtroom or division and the address at which it will be held on the next four blank lines.

 

Colorado Order Appointing Court Visitor JDF 809 Step 13: Indicate with a check mark whether the visitor fee is the responsibility of the petitioner, to be submitted to the court and paid at state expense or to be determined at a later date by the court. The judge, magistrate, probate registrar, or (deputy) clerk of court should sign and date the last two blank lines.
