INSTRUCTIONS: CALIFORNIA ORDER AUTHORIZING CONSERVATOR TO GIVE CONSENT FOR MEDICAL TREATMENT (Form GC-385)

 

 

This form is used by a California conservator to request authorization to give consent for medical treatment for their conservatee. This document can be obtained from the website of the California Judicial Branch.

 

California Order Authorizing Conservator To Give Consent For Medical Treatment GC-385 Step 1: In the first box at the top of the page, enter the name, state bar number, address, telephone number, fax number and the represented party of your attorney. If you do not have an attorney, enter the applicable information for yourself in this space.
