California Confidential Guardian Screening Form GC-212 Step 1: In the first blank box, provide all information requested about your attorney. If not represented by an attorney, enter all applicable information concerning yourself.
California Confidential Guardian Screening Form GC-212 Step 2: In the second box, enter the county in which the court is located, its mailing and street address, the city and zip code, and the branch name.

California Confidential Guardian Screening Form GC-212 Step 5: In the fifth box, enter the hearing date and time.
California Confidential Guardian Screening Form GC-212 Step 6: In the sixth box, enter the department.
California Confidential Guardian Screening Form GC-212 Step 7: Indicate with a check mark whether this concerns guardianship of a person or of an estate.
California Confidential Guardian Screening Form GC-212 Step 8: On line 1a, enter the name of the proposed guardian.
California Confidential Guardian Screening Form GC-212 Step 9: On line 1b, enter the date of birth of the proposed guardian.
California Confidential Guardian Screening Form GC-212 Step 10: On line 1c, enter the Social Security number of the proposed guardian.
California Confidential Guardian Screening Form GC-212 Step 11: On line 1d, enter the driver's license number of the proposed guardian and the state in which the license was issued.
California Confidential Guardian Screening Form GC-212 Step 12: On line 1e, enter the home, work and other phone numbers of the proposed guardian.
California Confidential Guardian Screening Form GC-212 Step 13: Answer the questions on lines 2 through 19 by checking "I have" or "I have not" as applicable. Provide additional documentation as instructed on each line.
California Confidential Guardian Screening Form GC-212 Step 14: On lines 20 through 22, provide all contact information requested concerning the minors.
California Confidential Guardian Screening Form GC-212 Step 15: Sign and date the form where indicated. Each proposed guardian must fill out and file a separate form.
