INSTRUCTIONS: ARIZONA RESIDENT PERSONAL INCOME TAX RETURN (SHORT FORM) 2011 (Form 140A) Arizona residents may use a form 140A to file and pay their state income tax provided their taxable income from the state is no greater than $50,000. If their income exceeds $50,000 a form 140 must be filed instead. Both documents are available on the website of the Arizona Department of Revenue. Arizona Resident Personal Income Tax Return (Short Form) 2011 140A Step 1: Enter your name on line one and your Social Security number. If filing jointly with a spouse, enter theirs as well. Arizona Resident Personal Income Tax Return (Short Form) 2011 140A Step 2: Give your home address on lines two and three. Check one of the boxes on lines four through seven to indicate your filing status. If claiming exemptions related to age, blindness or dependents, enter the number of each claimed on lines eight through 10. If claiming dependents, you must complete Schedule A on the second page. Arizona Resident Personal Income Tax Return (Short Form) 2011 140A Step 3: On line 11, enter your federal adjusted gross income. This will calculate your Arizona Gross Adjusted Income, which should be entered on line 18. Subtract the sum of lines 19 and 20 from the number on line 18. Enter the result on line 21. Follow instructions on lines 22 through 24 to further determine your reduced tax on line 25.
