INSTRUCTIONS: ALABAMA DEPARTMENT OF DEFENSE DEPENDENT SCHOOL SERVICE PURCHASE
 
Active and contributing members of the Alabama Teachers' Retirement System who were formerly part of the department of defense may purchase service by filing two documents. One, a statement of service, can be completed by you. The other form is a certification of service which must be completed by employees of the National Personnel Records Center and the Office of Personnel Management. Both documents can be obtained from the website of the Retirement Systems of Alabama.
 
Alabama Department Of Defense Dependent School Service Purchase Step 1: On the first blank line of the statement, enter your first name, middle or maiden name and last name.
 
Alabama Department Of Defense Dependent School Service Purchase Step 2: On the second blank line, enter your Social Security number.
 
Alabama Department Of Defense Dependent School Service Purchase Step 3: On the third blank line, enter your date of birth.
 
Alabama Department Of Defense Dependent School Service Purchase Step 4: On the fourth blank line, enter your home telephone number, including the area code.

Alabama Department Of Defense Dependent School Service Purchase Step 10: Check the fourth box if receiving a retirement benefit from any other entity that is based in part or whole on this service.
 
Alabama Department Of Defense Dependent School Service Purchase Step 11: Sign and date the form.
 
Alabama Department Of Defense Dependent School Service Purchase Step 12: Part II should be completed by a notary public.
